When it asks for a password, press select, then enter 7444 as a password. that will reset the password. Then, enter a new passcode. You can also, use the parental level menu, Change the level to anything you want, or turn it off.
Whatever you do, rememeber to eject the disk BEFORE you turn off the console, otherwise the ps2 will not save your changes.
Mine used to do that as well. But I think code is set as default to 000 so just enter that when it asks.
