There are 3 methods to format the phone

IMPORTANT: back up your phone and memory card first!

First: if the phone menus work there is a; go to menu -> tools -> settings -> general -> factory restore.
note: the defualt code is 12345
Seconed method requires you to enter these into the dial number area.

Third: is a button combination the needs to be done during the phone powering on
Hold down. 3, * and Green Call button
While holding this combination press the power button once to start the phone
Note: this does not require any codes; if this works the phone will prompt you to be set up again

There are two methods to format the memory card
First: Go to menu -> tools -> utillites -> memory -> options -> format mass storage
Seconed: Connect your phone to your PC via the supplied USB cable.
Then select Mass Storage mode on the phone.
Once you PC has installed the Mass Storage device you can format the card from My Computer.
Note: assuming Microsoft Windows XP or VISTA.
